Geriatric Home Visit Program
Many older patients who need to see their physician are unable to do so because they cannot leave their homes. If an older adult is housebound, Bridgeport Hospital's Geriatric Home Visit Group can bring the care they require right into their home.
The Geriatric Home Visit Group accepts patients who live in Bridgeport, Fairfield, Trumbull, Stratford, Shelton, Milford, Southport, Westport, Orange, Easton or Monroe.
The Home Visit Geriatrician becomes the primary care physician for these patients. (This means that the patient or family needs to notify the present primary care doctor about the changeover to the Home Visit Program.)
The services include a comprehensive geriatric assessment and subsequent follow-up visits by a geriatrician or a geriatric nurse practitioner. Home visits alternate between the physician and the nurse practitioner, so both healthcare providers get to know their patients.
If hospitalization becomes necessary for Home Visit Group patients, Bridgeport Hospital provides care. Hospital specialists (physicians specializing in the care of patients who are in the hospital) watch over the patient while they are in the hospital, under consultation with geriatric caregivers from the Home Visit Program.
